Additional Details:

Required Skills/Experience:

• Home health experience required.
• Minimum of one (1) year of experience in acute, subacute or home care setting.

Required Credentials: Graduate of an accredited speech and language program with a minimum of a Master’s degree.

Unit Details:

• Home care role traveling to patient homes and facilities as indicated.
• Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
• Instructs in communication and safe swallow, cognitive training as indicated.
• Develops and instructs in a therapeutic exercise program.
• Implements a program to restore function and achieve maximum functional status.
• Provides discharge planning and referral to resources as needed.
• Assumes responsibility for case management.
• Use of personal vehicle is required for home visits.

Shift & Scheduling:

• Mon-Fri.
• No weekends.
• No on-call.
