Required Skills/Experience: New grads okay as long as they have Level II experience and not just Level I observation experience. LTC or rehab experience required.
Unit Details: Role includes evaluating and rehabilitating patients with physical functioning disorders and/or wound care needs and developing plans of care in accordance with regulatory and clinical practice standards. Uses standardized assessments and evidence-based practice to support clinical interventions. Performs manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive device training, and wound care. Assesses patient progress, adjusts treatments accordingly, evaluates outcomes, and makes referrals in relation to outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Completes discharge summaries, documents in the electronic documentation system, records treatment minutes, obtains necessary DME and supplies, and may supervise PTAs, rehabilitation aides, and/or students.
Shift & Scheduling: 35 hours per week. No weekends. No call.
