Additional Details:

References: At least one reference must be from the same unit/specialty as the job you're applying to.

Required Skills/Experience: 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.

Required Credentials: Dementia testing will be required prior to start.

Special Requests: Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location. Must disclose previous employment on your resume or elsewhere in the submission for a DNU check. Backouts or quits with less than 14 days' notice incur a $500 fee.

Unit Details: 2 nurses (typically 1 LPN-12 hour shift and 1 certified medication aide-10 hour shift) per 48 residents; 1 LPN at NOC. 1 RN unit manager also present during day 8-10 hour shift. Common diagnoses/types of patients: Dementia, heart disease, COPD, CKD, depression.

Shift & Scheduling: Every other weekend and holiday. No on-call or call-back available.
