Required Skills/Experience:
Knowledge of and ability to apply community health principles and practice.
Working knowledge of home care reimbursement and regulatory guidelines.
Preferred Skills/Experience:
Proficiency with OASIS is highly preferred.
Infusion therapy experience preferred.
Experience with self-scheduling of caseload preferred.
Required Credentials:
Current driver's license and insurance required.
Special Requests:
Traveler housing within 50 miles and a 45-60 min or less commute from the facility is preferred.
Traveler candidates must have their own vehicle. This is a firm requirement.
Must be able to drive an automobile in inclement weather.
All scheduling requests, including RTO, must be made at the time of submission; the facility will not approve requests made at or after the offer or during the interview, and requests after the offer may lead to a rescinded offer.
Unit Details:
Provides and documents nursing care for sick and injured patients at home, including interventions to promote, maintain, or restore health, prevent illness, and support rehabilitation.
Coordinates care at home with other healthcare providers, professionals, and community representatives and evaluates individuals' and families' responses to interventions.
Care is provided independently and requires independent action and judgment.
Shift & Scheduling:
Willing to be flexible regarding work hours.
Every 3rd weekend/holiday coverage if required.
No more than 5 days of RTO will be accepted (less is better).
Travelers may not take all holidays off and must work two blocks (if scheduled/needed): Thanksgiving Day/Friday after and weekend after; Christmas Eve through Christmas Day and weekend after; New Year's Eve through New Year's Day and weekend after.
Must be willing to accept on-call responsibilities.
