Additional Details:
Preferred Skills/Experience: Home health experience preferred but not necessary.
Special Requests: Cannot have been employed by UnityPoint within the last 12 months. Taxable address must be more than 50 miles from the facility.
Unit Details: Average 4-6 patients per day. Patient population consists mostly of patients needing short-term therapy after hospitalization or a nursing home stay. Coverage area is within 50 miles of Fort Dodge, IA; this assignment averages 200-250 miles per week. Weekly team meetings are virtual; monthly meetings are in person.
Shift & Scheduling: Mon-Fri with no weekends or call. Required to work 1-2 of 3 summer holidays: Memorial Day, Independence Day, and Labor Day.
