Resume: Each employment history entry must include all the following: detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, teaching facility. References: At least one reference must be from the same unit/specialty as the job you're applying to. Skills Checklist: Cannot contain any N/As; all lines must have a numerical rating.
Special Requests: Any employment gaps greater than 30 days must be explained. Must disclose previous employment on your resume or elsewhere within the submission for a DNU check. Previous Sanford Health employment requires an end date at least 6 months before submission. Must take a full 6 months off from any Good Samaritan Society or Marshfield Clinic Health System location before submission.
Unit Details: Provides physical therapy interventions, evaluations, treatment plan development and modification, documentation, and patient/family education including home exercise programs. Treats patients of all ages with varying diagnoses and disorders. Uses modality equipment. Does not work alone.
Shift & Scheduling: Mon-Fri. No weekends. No call.
Floating Requirements: Primary setting is outpatient hospital-based; may cover inpatient, sub-acute, and skilled nursing facility settings.
