Travel Allied (Physical Therapist) - Home Health

Required Skills/Experience:

• Certified home health experience including proficiency with OASIS required.

Required Credentials:

• Home health certification required.
• Valid driver's license required.

Special Requests:

• Client prefers a 45 min or less commute from TOR Office (165 Tor Ct, Pittsfield, MA 01201).
• Must have own vehicle for this assignment.
• All RTO and scheduling requests must be made at submission and verified at upload; requests after upload or at/after offer will not be approved and may lead to a rescinded offer.

Unit Details:

• Provides direct patient care in home health, including collaboration with team members and participation in development of the patient's comprehensive plan of care.
• Uses rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities.

Shift & Scheduling:

• Rotating weekends. Holiday coverage required if needed.
• No call.
• No more than 5 days of RTO will be accepted.
