Required Credentials:

• Valid driver's license and appropriate auto insurance required.

Unit Details:

• Home health role providing evaluation and rehabilitation intervention in the home setting, including patient and family education.
• Evaluates and establishes plan of care to meet patients' needs in the home environment and provides direct patient care based on the established care plan and physician orders.
• Includes 30-day functional assessments and CMS required OASIS scoring.
• Uses point-of-care documentation in direct patient care settings.
• Oversees Occupational Therapy Assistant services and performs supervisory visits; takes supervision of CNA when other disciplines are no longer involved in the care plan.

Shift & Scheduling:

• Mon-Fri. Occasionally evenings or weekends are needed.
