Additional Details:
Required Credentials: 15 hours of approved continued education annually
Special Requests: You must be able to complete labs within 48 hours of signing. In addition to a valid driver’s license, you MUST include a proof of address document to apply. The document must be in your name & match the address on your driver’s license (and issued within the last 3 months). Any document other than those listed below will NOT be accepted: • Cell Phone Bill • Electric, Gas, Water, Sewer, or Wi-Fi • Mortgage Statement or Lease Agreement
Shift & Scheduling: Shifts can range between 8am and 6pm depending on the rotation within the week. Typical morning shift is 8am to 4:30pm and evening shift is 9:30am to 6:00pm
