Additional Details:

Required Skills/Experience:

  • 2-year acute care hospital case manager experience performing care coordination and discharge planning; hospital experience is a must.
  • Behavioral health-only experience is not accepted
  • Computerized documentation required.
  • Proficient in Microsoft applications.

Required Credentials:

  • Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests:

  • RTO must be listed at time of submission.
  • All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
  • References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
  • You must be able to complete labs within 48 hours of signing.
  • First-time travelers accepted; clinical will screen first-time travelers.

Unit Details:

  • Adolescent, adult, and geriatric patient population.
  • Admission Criteria (InterQual) (ED only).
  • Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc.
  • Ability to organize and participate in family meetings.
  • Participate in the development of the treatment plan.
  • Experience with HMOs, IPAs, etc.
  • Competent with arranging DME, SNF, and ambulance transport; must be able to perform this task independently.

Shift & Scheduling:

  • Every other weekend.
  • Weekend coverage must meet department needs.

Details:

AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment.
