Required Skills/Experience:
Minimum of one year of experience working with the frail, elderly, or long-term care population.
Preferred Skills/Experience:
Minimum of one year home care experience preferred.
Minimum of one-year wound care experience preferred.
Required Credentials:
RN nursing diploma or degree from an accredited School of Nursing or University.
Preferred Credentials:
Bachelor’s degree in Science of Nursing preferred.
Special Requests:
Reliable transportation, current valid Michigan driver's license, and car insurance required.
Unit Details:
Provides clinic and home-based direct care, including regular home visits.
Responsibilities include triaging emergent situations; monitoring acute and chronic medical conditions in coordination with primary care providers; administering treatments, procedures, and medications; wound care; lab draws; and nursing assessments.
Uses TruChart.
Shift & Scheduling:
Mon-Fri.
May work beyond normal working hours and on weekends and holidays, as necessary.
