Required Skills/Experience: Must have 1 year certified home care experience. 1 year case management experience. Ability to travel to cases as assigned.

Special Requests: required to perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Start date based on credentialing clearance. If not cleared by posted start date-a rescheduled start date will be necessary

Shift & Scheduling: Weekend rotations are once every 6 weeks
