Unit Details:
- Coverage includes Rutland and Bennington County.
- Home health role with OASIS and PT assessments, plan of care development, skilled physical therapy treatment, caregiver education, care coordination, and OASIS time-point documentation.
- Supervises PTAs, PT students, and home care aides.
- May perform PT/INR and blood pressure assessments, staple/suture removal, and wound care.
Shift & Scheduling:
- Mon-Fri with 2-3 weekend days.
