Travel Nurse (RN) - Case Management

Required Skills/Experience:
Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities.
Minimum 2 years of experience with acute inpatient hospital discharge planning.
Inpatient case management is required.
Experience with subacute patients required.

Required Credentials:
BSN required.
RN license required.

Preferred Credentials:
CCMC preferred.

Unit Details:
Coordinates patients through the acute care hospitalization.
Manages all aspects of discharge planning/transition process.
Performs utilization management and quality screenings.

Shift & Scheduling:
Rotating weekends: Every 4th or 5th weekend.
Holidays: One summer and one winter holiday.
No call.

Floating Requirements:
Floating is as needed.
