An interview with the unit manager may not be available prior to offer. If you do not interview with the unit manager, you may interview with a third-party clinical nurse manager.
Preferred Skills/Experience:
2+ years of home health experience preferred.
Required Credentials:
An associate degree or certificate is required.
Active WI licensure is required, in addition to MI licensure.
A valid driver's license is required.
Unit Details:
The role averages 5 visits per day and could include up to 6 visits, depending on the territory, patient census, and staff availability. Travel may reach a maximum of 150 miles in a given day, but not all days.
The role includes OASIS admissions, all time points, episode management, doctor calls, coordination of care, timely EMR documentation, and clinical competencies in wound care, IVs, ports, lab draws, and chronic disease management.
Shift & Scheduling:
Day shifts may be 8-10 hours.
The schedule includes every 3rd weekend and holiday rotation; weekend coverage could change depending on the nursing pool for needed coverage.
On-call is every 4th weekend and 1 day during the week.
The assignment includes on-call coverage for the 4th of July.
