OK Licensure by Endorsement

Process

Permanent License

  1. Submit an application online and pay the fee. You may also mail in an application. The process for RNs and LPNs is the same.
  2. You can find the SSL application instructions here and the MSL application instructions here
  3. You must provide a Social Security number on the application. Submit name change documentation, if applicable.  Provide your full legal name with the notation “NMN” if you have no middle name.
  4. Provide proof of citizenship by submitting a notarized Evidence of Status form and necessary supporting documentation.
  5. Provide a background check with fingerprints.
  • If you have an Oklahoma mailing address, you will need to make an appointment online and then go to an IdentoGo center sponsored by Idemia located within the state to provide fingerprints electronically.
  • You can find Fingerprinting FAQs and further instructions here
  • If you have an out-of-state mailing address, the Board staff will mail you specific fingerprint cards that you must take to a local law enforcement agency to obtain the fingerprints. You must wait to receive the specific coded Form RS-04 fingerprint cards from the Board office prior to obtaining your fingerprints. An addressed envelope will be provided as the fingerprint cards must be mailed by the law enforcement agency directly to an IdentoGO center..
  1. Include a disclosure of any criminal charges, disciplinary action, or judicial declaration of mental incompetence, if applicable.
  2. Request verification of licensure via Nursys from your original state of licensure. If this state does not participate in Nursys, you will need to contact that state's BON and request verification be sent to the Oklahoma BON. Verification of licensure must indicate that you have passed the licensure examination.
  3. If your state does not fully verify your nursing education, the Board will request that an official transcript be submitted to this office directly from the nursing program.
  • An official transcript from the nursing education program must be submitted directly from the nursing education program. The transcript must show the diploma or degree and the date conferred. The transcript must be obtained from an entity approved and recognized by the U.S. Department of Education as a primary source for providing education transcripts
  1.  Demonstrate evidence of meeting continued qualifications for practice by having your most recent employer complete the Employment Verification Form. If you have not practiced as a licensed nurse for at least 520 hours in the last five years, an official transcript or certificate of completion for a refresher course approved by the Oklahoma Board of Nursing must be sent directly to this Board from the program.

Temporary License
