    Additional Information

    Required Credentials:

    • Dementia testing required prior to start.

    Required Skills/Experience:

    • 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.

    Special Requests:

    • At least one reference must be from the same unit/specialty as the job you are applying to.
    • Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location. Must disclose previous employment on your resume or elsewhere within the submission for a DNU check.
    • Each employment history entry must include detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, and whether the facility is a teaching facility. Any employment gaps greater than 30 days must be explained.
    • Skills checklist cannot contain any N/As; all lines must have a numerical rating.

    Unit Details:

    • Uses Home Health HCHB and Net Health.
    • Productivity expectations: 90% or 28 points for home health (full time) and 88% for SNF.
    • OT available onsite for supervision; therapy supervisor onsite.

    Shift & Scheduling:

    • Schedule options: Mon-Fri, Sun-Thu, or Tue-Sat.
    • No call or call back available.
    • Work week runs Sunday through Saturday.

    Floating Requirements:

    • Primary setting is Grand Island SNF and the surrounding area for home health; occasionally will cover Hastings SNF or home health for PTO or census.
