Required Skills/Experience:
- Must have some home health experience.
- Must have 1 year post grad PT experience.
- Must have Ortho experience.
Preferred Skills/Experience:
- Prefer post grad home health experience.
Special Requests:
- Any employment gaps greater than 30 days must be explained.
- The facility may make one-off exceptions if previously employed at the facility, but exceptions must be approved by St. Luke's Health System.
Unit Details:
- In-home therapy with admissions, existing patients, and recertifications; includes a few assisted living patients.
- Multidisciplinary adult home health caseload; approximately 78% Medicare patients, primarily elderly, rehab, CVA, and inpatient surgical referrals including ortho, surgical, and wound patients; no lymphedema.
- Limited oxygen requirements.
- 4-6 patients per day.
- Can start shift from home and do not need to stop in an office.
- Coverage area up to 60 miles east and west, about 60 miles north to Sun Valley, and about 40 miles south to the Idaho/Nevada border.
- Monday and Wednesday multidisciplinary meetings to discuss current admissions.
Shift & Scheduling:
- Mon-Fri.
- No weekend requirements.
- No holiday requirements.
