    Additional Details:

    All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

    References must cover at least 1 full year of relevant experience in specialty within the past 3 years

    You must be able to complete labs within 48 hours of signing.

    ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)

    Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

    Required Skills/Experience:

    • 1 year of skilled home health experience.
    • OASIS experience required.

    Required Credentials:

    • Cannot have any hits/suspensions on license or any background hits from any point in the past.

    Special Requests:

    • RTO must be listed at time of submission.
    • RTO restrictions: confirm with hiring manager.
    • Cannot be currently or recently (within 6 months) employed with network; must have left in good status if a former employee.
    • Traveler is expected to have their own transportation.

    Unit Details:

    • 1:1; may visit up to 6 patients per day; 30 productivity points standard.
    • System does a good job grouping clients, minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
    • Uses Rover (phone app) and OASIS.
    • Adult and geriatric patient population.
    • Patient population includes dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke.
    • Parking Cost: Free

    Shift & Scheduling:

    • No weekends.
    • No call.
