Required Skills/Experience:
Float experience does not count towards the unit experience requirement.
Home Health experience required.

Special Requests:
Any overtime must be pre-approved and requires written approval from the administrator.

Unit Details:
Coverage area includes Sonoma and Marin.
Average 4-6 visits per day, depending on type of visit, for PT/OT/SLP.
The productivity expectation is 28-32 points per week for PT/OT/SLP.
Productivity point values are: SOC: 2.5 points; ROC: 2 points; Evaluations/Recert/Discharge OASIS: 1.5 points; Discipline DC, routine visit, and 30-day reassessment: 1 point; weekly case conference: 1 point.

Shift & Scheduling:
Monday-Friday.

Floating Requirements:
May be asked to help in other territories within the facility's service area as needed.
