Additional Details:
Required Skills/Experience:
- Float experience does not count towards the unit experience requirement.
Required Credentials:
- Dual LIC MA/NH or NH LIC only is okay.
Unit Details:
- Average 4-6 visits per day depending on type of visit.
- Productivity is 28-32 points per week for PT/OT/SLP. SOC: 2.5 points, ROC: 2 points, evaluations/recert/discharge OASIS: 1.5 points, discipline DC, routine visit, 30-day reassessment: 1 point, weekly case conference: 1 point.
Shift & Scheduling:
- Monday-Friday.
Floating Requirements:
- Coverage includes Methuen, Haverhill, Salem, NH, and surrounding areas. May be asked to help in other territories within the facility's service area as needed, about a 60 mile radius from the facility address.
