- Next-day mandatory reporting of start-of-care visits.
- Clinical notes and other documentation are due within 48 hours of the visit.
- Territory: Greater Sacramento, CA; may be required to expand as needed.
- Documentation system: Kinnser.
- Equipment issued: iPad, Verizon One Talk.
- Productivity requirement: 30 points per week.
- Point-to-hour conversion: 1 = 1.33 hrs.
- Visit types and point values: Start of Care 2; Recertification, Resumption of Care, OASIS Discharge, Evaluation 1.5; Revisit, non-OASIS discharge 1.

Required Credentials:
Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.

Unit Details:
Assesses referred patients and plans and administers physical therapy intervention plans; schedules treatments; completes treatment documentation; assigns assistants and aides; instructs patients, families, hospital staff, and therapy students; regularly assumes Charge Therapist duties.
Includes pediatric physical therapy care with assessment and intervention in gross motor skills, gait, therapeutic exercises, development, and durable medical equipment.

1 weekend a month.

- Uses ReDoc for outpatient documentation.

- Mon-Fri. No weekends.

Floating Requirements:
- May be required to float to Adventist Health locations within 30 miles.

Territory includes Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Palo Alto, Portola Valley, Santa Clara, Stanford, Woodside, Sunnyvale, and neighboring cities as needed.

Shift & Scheduling: Schedule can be flexible based on patient's availability. One weekend day per month.
