Required Skills/Experience: 1 year Certified Home Care experience required. Experience in hospice, private care, or working as a case manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. 1 year Case Management experience required.

Required Credentials: Contingent/temp licensure is not accepted. Primary Source Verification of licensure/certification is required for application.

Unit Details: Home health role with travel to cases as assigned. Includes admission processes, case management responsibilities, and timely electronic documentation and interdisciplinary communication.

Shift & Scheduling: Weekend rotations are once every 6 weeks.

Floating Requirements: Coverage area includes Chelsea, East Boston, Revere, and Winthrop.
