    Required Skills/Experience: 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.

    Required Credentials: Dementia testing will be required prior to start.

    Special Requests: Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location. Must disclose previous employment on your resume or elsewhere within the submission for a DNU check. Any employment gaps greater than 30 days must be explained.

    Unit Details: Post-acute setting. Uses Net Health and HCHB. Supervises 2 assistants. Productivity expectations: SNF 82%; home health 95%, including drive time and documentation time. Full-week home health productivity is 27 points, based on visit type; routine visits are 80 min and evaluations up to 120 min. Home health work includes travel between locations and from patient to patient.

    Shift & Scheduling: Mon-Fri, with possible shifted schedules Tue-Sat or Sun-Thu. No on-call or call back.

    Floating Requirements: Covers both SNF and home health, varying week to week at approximately 50% each; primarily Grand Island, with possible assistance in Hastings or Kearney to cover PTO or obtain full hours.
