Required Skills/Experience:
- 1 year of home health experience required.
Unit Details:
- Prioritize start of care visits.
- Supervise Physical Therapy Assistants.
- Start of care visits require next-day reporting.
- Clinical notes and other documentation are due within 48 hours of visit.
- Documentation system: Kinnser.
- Equipment issued: iPad, Verizon One Talk.
- Productivity requirement: 30 points per week.
- Point to hourly conversion: 1 = 1.33hrs.
- Visit types and point values: Start of Care = 2; Recertification, Resumption of Care, OASIS Discharge, Evaluation = 1.5; Revisit, non-OASIS discharge = 1.
Shift & Scheduling:
- Monday-Friday with one weekend per month rotation.
Floating Requirements:
- Territory: Sacramento, CA. Will be required to expand as needed.
