Required Skills/Experience: 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.
Special Requests: At least one reference must be from the same unit/specialty as the job you're applying to. Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location. Must disclose previous employment on your resume or elsewhere within the submission for a DNU check. Each employment history entry must include detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, and teaching facility. Any employment gaps greater than 30 days must be explained.
Unit Details: SNF and Home Health setting, approximately 50/50 but may fluctuate. Supervises 2 PTAs. Productivity expectations: 80% SNF and approximately 6 patients per day for Home Health (28 patients per week at 90% productivity if only in Home Health).
Shift & Scheduling: No on-call or call-back requirements.
Floating Requirements: SNF coverage may include St John's and St Luke's. Occasional coverage to Hastings or Grand Island for SNF or Home Health may be needed.
