Additional Details:
Required Skills / Experience: 2 years acute care hospital or 1 year acute + 1 year of Hospice, HH or other work with elderly
Required Skills: Drug & alcohol Screens (SBIRT), Mental Health Evals (SAFE-T), Capacity Evals, Behavior Plans, BH referrals, Provide or assist with Tele pysch evals, Discharge planning, End of life conversations, Single Bed Certs/ITA process, difficult placements, Prior experience in home health, hospice, SNF’s, Medicaid/Medicare, insurance authorizations.
Required Credentials: LICSW or LSWAIC (only if they do not need supervision by MHS)
