Additional Details:
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
References must cover at least 1 full year of relevant experience in specialty within the past 3 years
You must be able to complete labs within 48 hours of signing.
Required Skills/Experience: Will not accept social workers with experience specialized in behavioral health or ED.
Required Credentials: MSW required. Cannot have any hits/suspensions on license or any background hits from any point in the past.
Preferred Credentials: LCSW preferred.
Special Requests: RTO must be listed at time of submission.
Unit Details: Adult and geriatric patient population. Social workers are case managers/discharge planners. Care management department consists of RNs and MSW/LCSWs all performing discharge planning. Two clinical coordinators can assist with tasks that do not require an RN or MSW/LCSW license, such as sending referrals via Epic, following up with facilities to obtain a bed assignment, delivering DME to the bedside, and faxing clinicals to Livanta. Unit-based role responsible for initial assessment within 24 hours, coordination of discharge plans, and coordination of all post-acute care needs. Will work with a variety of diagnoses and work closely with the medical team, insurance case managers, and Providence UM case managers to safely and quickly discharge patients to the lowest level of care possible.
Shift & Scheduling: Every other weekend. Sunday through Thursday schedule, subject to change depending on staffing. RTO: No more than 7 days.
Floating Requirements: Floating expectation: Laguna Facility.

• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
