Certifications are required to be included at time of submission.
Required Skills/Experience: New grads okay as long as they have Level II experience and not just Level I observation experience. LTC or rehab experience required.
Unit Details: Long-term care/rehab setting for patients with cognitive and/or ADL physical functioning disorders. Duties include screening and evaluations, plan of care development, standardized assessments, rehabilitative treatment, ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices. Documents in the electronic documentation system, records treatment minutes, provides discharge summaries, and provides consultation/counseling to patients, families, caregivers, and other service providers. Able to supervise and support occupational therapist assistants, rehabilitation aides, and/or students.
Shift & Scheduling: Mon-Fri. 8 hr days; times are flexible as long as they are consistent and communicated with the DOR. Start time is before 6AM and no later than 10AM. No call. No weekends unless a holiday falls within Mon-Fri, then hours may be made up on the weekend.
