    Unit Details:

    • Home health setting with an average visit length of 45 minutes.
    • Screens, evaluates, re-evaluates, and treats patients; develops treatment plans; documents evaluations, treatment goals, and objectives; teaches patients and family members; and coordinates care with the interdisciplinary team.
    • Broad independence while working as part of an interdisciplinary patient-care team.
