The Process

Process

1. Submit an application for Licensure by Endorsement online

  • You must provide a valid United States Social Security Number

2. Provide a minimum of 960 practice hours at the level of licensure sought. You will include current or previous employers and the dates you worked for them in the application. 

* Only needed if it has been more than five years since you graduated from your nursing program.

3.Pay  the associated fees


After you submit your application and payment, you will receive an email with the next steps.


4. Complete fingerprints for national criminal background check

5. Official and final transcripts

  • Option A: Contact your school and request that it send official sealed final transcripts that include your graduation date and degree awarded to the OSBN office.
  • Option B: If your school subscribes to a national document transfer network, you may request the service to send official electronic transcripts to OSBN at: osbn.transcripts@state.or.us.​

6. Verification of your original AND current (or most recent) state license verification

  • If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Oregon as the receipt of the verification.
  • If the state does not participate in Nursys, you must contact their state Board directly for further instructions. You will need to send them a Verification of Licensure form to complete and submit to the Oregon Board of Nursing on your behalf.

7. If you are a non-US nursing program graduate, you must have your nurse education evaluated for equivalency to UC standard by one of the OSBN-approved credential evaluation agencies(if applicable).

Renewal License

Timing
  • Renewal of your license occurs every 2 years
  • Your license expires at 12:01a on your birthday in either an odd or an even year (based on if you were born in an odd or even year). 


Process
  • Within 60 days - 
  • Complete renewal application online
  • Pay fee of $100.00
  • 61 days or more - 
  • Complete application to reactivate your license
  • Pay fee of $100.00 + $50.00 each additional day + $264.00 for reactivation
  • Provide proof of 960 hours of nursing practice within the last five years

*If you cannot meet the practice hour requirement, you must complete a nurse re-entry program while under limited licensure in order to be eligible to reactivate your license.


Timing

Permanent License

Approximately 3-6 weeks for a license. The Oregon Board of Nursing does not provide a definitive timeline since it is dependent upon various factors.

  • Oregon Nursing Advancement Fund Surcharge: $9.00

Important things to Know About Applying For Licensure for Endorsement

  • If you are the spouse or domestic partner of an active service member, the OSBN may expedite your licensure endorsement process.
  • The Oregon State Board of Nursing does not issue plastic license cards.  Employers and nurses can verify licenses through the online license verification system, or the national Nursys system.
