The Process

Process

Temporary License

1. Fill out the application for Licensure by Endorsement (print legibly in black ink)

  • Answer all the questions. If not applicable, indicate “N/A”
  • Sign and date application 
  • Indicate a valid United States Social Security Number

2. Complete a Self Query Report - through the National Practitioner Data Bank

  • The Board of Nursing will accept either the original hard copy that is mailed to you or the original emailed report. You will need to attach the PDF file in the email. Please forward the original email to nursing@dcca.hawaii.gov. 

3. Pay the associated fees

  • Attach a personal check, money order, or cashier's check for the appropriate amount made payable to: COMMERCE & CONSUMER AFFAIRS.

4. Mail in your completed application to the Hawaii Board of Nursing

5. A completed original Verification of Employment form which must first be signed by your employer in Hawaii. Letters of hire will not be accepted.

6. Verification of all states where you hold or ever held a license (even if they are expired)

  • If the state(s) participate in Nursys, you must complete the Nursys verification process, pay their fee and designate Hawaii as the receipt of the verification. Instant Nursys searches are not accepted.
  • If the state(s) does not participate in Nursys, you must contact their state Boards directly for further instructions. They will need to submit an official Verification of License form to the Hawaii Board of Nursing on your behalf. You only need proof of mailing the verification form (a receipt of certified mail, copy of the canceled check for the verification fee)
  • If you hold a license in a non-Nursys participating state and it is NOT your original state of licensure, you can just use their online system and print out your license verification and mail that in. 

7. Submit to a FBI fingerprint check through the Hawaii Criminal Justice Data Center (HCJDC). 

  • To obtain a FBI national Criminal History Record check and the State of Hawaii Criminal History Record check, you will make an appointment online
  • Fieldprint code that you must enter is FPHIBrdNursing (not case sensitive)
  • And then you will be fingerprinted electronically at any Fieldprint Inc. location nationwide or any other fingerprinting agency approved to send electronic fingerprints to the Hawaii Criminal Justice Data Center (HCJDC). 
  • A license application must be filed within 30 days of the fingerprinting to ensure that the results are obtainable from the HCJDC. If the results are not obtainable, you will be required to obtain new fingerprints.
  • The results take 3-4 days to get to the Hawaii Board of Nursing but you can receive a temporary license while your background check is processing. 
  • If you are in a location that does not have Field Print, you can ask for hard copy fingerprint cards via email to the HI Board of Nursing. The HI Board of Nursing will then email you instructions and mail a packet.

8. Submit documentation requirements if you answered "yes" to any of the background questions (if applicable).

Renewal License

Renewal of your licensure occurs every 2 years

  • All licenses, regardless of issuance date, expire on June 30 of each odd-numbered year and are subject to renewal. 

PROCESS

  • Renewal applications are made available about 60 days prior to the license expiration date.
  • Required contact hours: choose one option from the following 10 options
  • You may be selected at random to complete a Complete Background Check if you were licensed in Hawaii before July 1, 2017.

Online renewal of a nurse license is available to licensees who have not had any disciplinary action pending or taken or have not had a conviction during the two years prior to the renewal date.

Timing

You can appear in person to receive a temporary license the same day (Monday - Friday  7:45a-4:30p). Everything must be submitted before walking in. The only thing you need to bring in with you in the temporary permit application and the associated fee. The temporary license is valid for 90 days.

Approximately 15-20 business days upon receipt of all required documents. The temporary license is valid for 90 days.

If your license will be issued between July 1, odd-numbered years (2015, 2017, 2019) and June 30, even-numbered years (2016, 2018, 2020) you pay $234.00

If your license will be issued between July 1, even-numbered years (2016, 2018, 2020) and June 30, odd-numbered years (2017, 2019, 2021) you pay $166.00

Important things to Know About Applying For Licensure for Endorsement

  • You are allowed only one temporary permit. 
  • While permitted you are only allowed to practice nursing only if employed by employer indicated on your Verification of Employment form. Once your permit is issued, no other will be reissued in the care of another employer. 
  • The Board of Nursing will only issue a temporary license when they receive all the above. A temporary license is used while the Board of Nursing is processing your information. 
  • A temporary license is issued under certain circumstances for instance, if you were educated outside the US and they are waiting for your credentials to be evaluated. Or if they are awaiting your licensure verification from a state that does not participate in Nursys.
  • Prior disciplinary action or conviction, which has not been expunged, must be reviewed by the board.
