    Additional Details:

    Individual professional liability coverage is mandatory for assignment duration (only individual malpractice insurance is accepted by the facility).

    Required Skills/Experience: Seven (7) years of experience is required, five (5) of which were in a supervisory role. Must have 2 years’ experience in discharge planning and utilization review.

    Required Credentials: NYS nursing license registration and a copy of physical license/diploma are required for submission. Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED). PRI Certification required.

    Preferred Credentials: Master's degree in Nursing preferred.

    Special Requests: If you've previously worked at an NYCHH facility, please update your employment dates on your profile and contact your Advocate with your NYCHH supervisor's name to speed up your application. This information is necessary for your submission. All time off and special requests should be discussed directly with managers upon arrival (no pre-approval). Clinicians must start assignments within 30 days of receiving an offer.

    Unit Details: Associate Director-level supervision to the Care Management Department. Transitional care/care management role focused on reducing 30-day readmissions for high-risk patients after discharge from inpatient med/surg units and the adult emergency department, supporting safe return to the community. Provides oversight of care coordination and direction to Accountable Care Managers (RNs), Care Coordinators (CLWs), clerical, and ancillary staff. Performs all duties of the Director of Nursing in his or her absence. Monitors patient care performance, assesses units with nurse managers, assists with corrective action, and develops record-keeping for medication errors/counseling.

    Shift & Scheduling: No on-call. Overtime not allowed at NYCHH facilities (requires written manager approval in rare cases).

    Clinician must complete Additional skills section of resume with detailed description of patient pops and procedures worked for each work history.
