    Additional Details:

    Required Skills / Experience: Must have Supervisory / Manager experience

    With at least 3-5 years of leadership experience

    Seasoned RN Case Management background

    Experience with the following patients: NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU.

    Case management and discharge planning, strong assessment skills, ability to discharge homeless patients and discharge patients to the next level of care to SNF, IP, LTAC, Shelter, Assisted Living Facility, Medicare/Medicaid patients

    Preferred Skills / Experience: Ideally with inpatient clinical experience.

    Unit / Facility Details: Case Load: As low as 15 patients and as high as 25-30, depending on staffing
