The Process

Process

  1. Mail in a completed, signed, and dated application for Licensure by Endorsement 
  2. Mail in the associated fees (check or money order)
  3. Mail in two 2x2 passport-type photographs
  4. You will need your full Social Security Number or a Signed Affidavit 
  5. Name change document- If the name on your application differs from the name on any of your supporting documents, proof of name change is required. Acceptable documents are: marriage certificate, divorce decree, court order or spouse’s death certificate. 󠆴 
  6. A copy of a government issued photo ID 󠆴 
  7. Criminal background check. Criminal background check instructions can be found on the Board of Nursing’s site under Criminal background check.
  8. Verification of your original state license by examination. If the original state is expired, verification is required from both the original and a current state. 
  • If the state participates in Nursys, you must complete the Nursys verification process, pay their fee and designate Washington as the receipt of the verification.

If the state does not participate in Nursys, you must contact their state Boards directly for further instructions. You will need to send them the Verification of Licensure form (p. 9 of the application) to complete and then they will submit it directly to the Washington State Board of Nursing on your behalf.

A temporary practice permit will automatically be issued to you if you do not indicate any prior conviction or discipline history.


Renewal License

Renewal of your license occurs every year

Renew your license online - paper applications are no longer valid

Required contact hours: 18 hours (Must include 2 hours for LGBTQ & 3 hours ofHIV/AIDS)

  • Contact Hours: Provide Course Completion Certificates
  • Academic Option: Provide transcript that indicates completion of an undergraduate or graduate course in nursing or relevant to the practice of nursing.
  • Teaching Option: Provide acceptance letter/email as evidence of having developed or taught a CE course or educational offering approved by the Board or a Board-approved accrediting body.
  • Author or Editor Option: Provide acceptance letter from publisher, as evidence that you authored or edited a book, chapter or published peer reviewed periodical

*All continuing education must be relevant to your current field of practice

Important things to Know About Applying For Licensure for Endorsement

  • An application that remains incomplete for ninety (90) days or more from the date of submission shall be considered abandoned, and closed by the Board.
