Lab Completion: All health-related requirements and lab work must be completed within 5 calendar days of signing.

You must be able to complete labs within 48 hours of signing.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Required Skills/Experience:

• Home health, case management, or clinic experience is required.

Preferred Skills/Experience:

• Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities is preferred.
• Community resource knowledge is preferred.

Special Requests:

• Reliable transportation is required, as travel may be necessary during the workday.

Unit Details:

• Adult and geriatric patient population.
• This is not a hands-on nursing role.
• The role supports a Transitions of Care program focused on reducing 30-day readmission rates.
• LPNs assist the RN Care Manager with implementing care plans and perform appointment scheduling, biometric data monitoring, patient education, patient visits, home visits, community-resource connection, and frequent patient contact.
• Work includes screening calls, follow-up appointment coordination, triage of barriers, medication-adherence education, scoring documents, and alignment with community services.
• Work is primarily remote and telephonic, with hospital-based work and possible visits to patients' homes or provider offices for observation or education.
• The LPN participates in virtual multidisciplinary care-team rounds with an RN, social worker, pharmacist, and other professionals and provides patient updates.
• The monthly rolling caseload is approximately 25–30 patients.
• The program lasts 30 days, after which goals are reassessed for closure or transition to long-term management.
• LPNs will not transport patients.

Shift & Scheduling:

• Monday–Friday.
• Saturday: No.

Floating Requirements:

• Floats to Rochester General Hospital, Unity Hospital, patient homes, and provider offices.
