Required Skills/Experience:
- Inpatient case management is required.
- Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities.
- Minimum 2 years of experience with acute inpatient hospital discharge planning.
- Experience with subacute patients required.
Required Credentials:
- BSN required.
Unit Details:
- Coordinates patients through the acute care hospitalization.
- Manages all aspects of discharge planning and transition process.
- Performs utilization management and quality screenings.
Shift & Scheduling:
- Rotating weekends: Every 4th or 5th weekend.
- Holidays: One summer and one winter holiday.
- No call.
