Required Skills/Experience: Inpatient Care Management, Acute Hospital, Case Management/Utilization Review, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Prior Authorizations, Regulatory, CMS: Centers for Medicare and Medicaid Services, DRG (Diagnosis Related Groups), HEDIS (The Healthcare Effectiveness Data and Information Set) Measures, HIPAA guidelines (Health Insurance Portability and Accountability Act), NCQA (National Committee for Quality Assurance), OSHA, The Joint Commission/ Core Measure/National Safety Goals. Needs recent acute care experience in a hospital setting.

RTO must be listed at time of submission.
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
References must cover at least 1 full year of relevant experience in specialty within the past 3 years
Cannot have any hits/suspensions on license or any background hits from any point in the past.
You must be able to complete labs within 48 hours of signing.
Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.
Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
Shift & Scheduling: Are weekends required? Yes every other weekend
Floating Requirements: AYA Healthcare professionals may be required to float to other units and facilities in order to meet the needs of PH&S. Facilities: Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent
Details:
• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
