Required Skills/Experience:
- 1 year Case Management experience.
- Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.
Required Credentials:
- Contingent/temp licensure is not accepted.
Unit Details:
- Travel to cases as assigned.
- Responsibilities include assessing patient conditions, implementing care plans, providing ongoing education and support to patients and their families, admission processes, case management responsibilities, and electronic documentation.
- Service area includes Chelsea, East Boston, Revere, and Winthrop.
Shift & Scheduling:
- Weekend rotations are once every 6 weeks.
