Additional Details:
Required Credentials:
- Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:
- RTO must be listed at time of submission.
- All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract. Candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
- References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
- You must be able to complete labs within 48 hours of signing.
- Permanent address must be at least 50 miles from the facility.
- Only 1 time off request per 13 week assignment of 5 days or less.
Shift & Scheduling:
- Shift times may vary and may be one of the following: 0700-1530, 0730-1600, 0800-1630, 0830-1700.
- No call.
- Every other weekend or 4 weekend days in a 4 week schedule.
Floating Requirements:
- Will float within scope to meet facility needs.
