Required Skills/Experience:
- Current in-patient case management experience.
Unit Details:
- Coordinates care for a select caseload from triage through care coordination during JHM management and safe transfer home.
- Proactively communicates with payors to ensure approval of treatment plans and required resources.
- Maintains and responds to clinical system and fiscal data related to specific patient populations.
- Collaborates with the multidisciplinary team and patient/family to support effective and efficient transition planning.
- Coordinates transition to post-acute settings including nursing facilities, acute inpatient rehab, and local shelters.
- Enters and maintains patient information in departmental systems per operational guidelines.
- Will not have access to or administer medications to patients.
- Uses NaviHealth for post-acute facility referrals.
Shift & Scheduling:
- No weekends, no holidays, no call.
