Additional Information
You must be able to complete labs within 48 hours of signing.
Required Skills/Experience:
- Minimum 1 year of acute-care hospital Case Management experience.
- Intermediate computer skills.
Unit Details:
- Adult and geriatric patient population.
- Uses MCG or InterQual; experience preferred.
- Manages complex hospitalized patients, coordinates services, facilitates timely transitions of care, and documents accurately in the electronic medical record.
Shift & Scheduling:
- Rotating weekend schedule.

- In addition to a valid driver’s license, you MUST include a proof of address document to apply. The document must be in your name & match the address on your driver’s license (and issued within the last 3 months).
