References: At least one reference must be from the same unit/specialty as the job you're applying to.

Unit Details: Caseload 15-20 patients per day. Works with the social work team consisting of 15 full-time and 17 part-time social workers. Contracts with 5 local facilities with 10 beds reserved at each facility to assist with complex placements; specific criteria must still be met, and financial assistance is limited to uninsured or underinsured patients. Receives a variety of complex patients and those with no other hospital options. Will consult with RT and Pharmacy as needed for discharge planning.

Required Skills/Experience: Acute Inpatient Case Management experience that includes at least 1 year in a LARGE Hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center) (this does NOT include telephonic, utilization management, or therapy experience),

Preferred Credentials: ADN

Shift & Scheduling: Mon-Fri. No weekends. Holidays rotated as needed.

Floating Requirements: Will cover all inpatient units throughout the hospital.
