ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)
Required Skills/Experience:
- 1 year of skilled home health experience required.
Preferred Skills/Experience:
- Disease management, PICC lines, catheter care, and education preferred.
- Community hospital experience is preferred.
Required Credentials:
- Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:
- RTO must be listed at time of submission.
- All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract; no exceptions. Candidates must be responsive and willing to go to a lab to complete health orders as soon as possible after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
- References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
- You must be able to complete labs within 48 hours of signing.
- Cannot be currently or recently (within 6 months) employed with the network; must have left in good status if a former employee.
- RTO restrictions: Confirm with hiring manager.
Unit Details:
- Adult and geriatric patient population.
- May visit up to 6 patients per day; 30 productivity points standard.
- Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
- Uses Epic Remote Client for Home Health on laptop and Rover phone app.
Shift & Scheduling:
- No weekend requirement.
- No on-call requirement.
- Orientation for all Oregon facilities is in Portland, OR for RNs, LPNs, and CNAs.
Details:
• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
