Unit Details:
- Home health RN case manager role serving the Bradford, VT; Randolph, VT; Springfield, VT; White River Junction, VT; and Lebanon, NH areas.
- Screens, evaluates, re-evaluates, and treats patients while coordinating and communicating treatment activities with the interdisciplinary team and the patient.
- Develops the plan of care with the physician, patient, family, or primary caregivers on admission; periodically reviews and recertifies the care plan as appropriate.
- Coordinates care through interdisciplinary communication and case conferences and advises other disciplines of discharges in a timely manner.
- Provides ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and progress toward goals and communicates significant changes to the physician and VNH colleagues.
- Documents evaluations, treatment goals, and objectives.
- Teaches patients and caregivers self-care techniques and provides medication, diet, and other instructions as ordered by the physician, including health counseling opportunities.
