Resume: Each employment history entry must include all the following: detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, teaching facility.

References: At least one reference must be from the same unit/specialty as the job you're applying to.

Special Requests:
Any employment gaps greater than 30 days must be explained.
Cannot apply to more than one job order on the same unit and shift at the facility.
Must disclose previous employment on your resume or elsewhere within the submission for a DNU check.
Candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity.
RTO needed upfront (7 or less days per 13-week contract). Additional RTO will not be approved, including after submissions.

Unit Details:
The nurse acts as charge, passes medications, and performs wound care.
Patient population includes heart disease, diabetes, and dementia.
There are no rehabilitation beds; the unit is LTC only.
Physical therapy is available as needed, and a social worker designee is available.
The pharmacy is off site.
After-hours physician availability is by phone.
Oxygen equipment includes concentrators and portable concentrators.

Shift & Scheduling:
Every other weekend.
Every other major holiday.
No call.
Self-scheduling is not available.

Floating Requirements:
First to float.
