    Additional Information

    Required Skills/Experience: You will be required to complete proctored testing prior to attending orientation.

    Preferred Skills/Experience: 1-3 years related work experience preferred.

    Special Requests: Cannot have worked perm, travel, or PRN for any Beth Israel facility within the past 6 months.

    Unit Details: Coordinates care for patients from referral/consultation to discharge/follow-up. Assists with the pre-authorization process. Serves as the contact and facilitator to receive consults and patient information. Coordinates patient scheduling, including consultation, diagnostic, and treatment procedures. Gathers information for history and physical. Performs and documents patient evaluations and assessments. Assists in ordering labs, x-rays, and diagnostic procedures. Works in collaboration with surgeons and physicians to educate patients and their families in medical care. Assists in developing patient/family educational materials and participates in patient rounds and case conferences. Serves as a liaison and clinical expert to Quality & Safety for skin- and wound-related serious reportable events. Leads NDNQI quarterly pressure injury prevalence survey efforts for the hospital. Uses web-based applications and Microsoft Office applications, which may include Outlook, Word, Excel, PowerPoint, or Access.
