Required Skills/Experience:

• Experience with discharge planning and utilization review.

Special Requests:

• Cannot have worked at any CommonSpirit, CHI, or Dignity location as a direct employee (full time, part time, or PRN) within the last 12 months; PRN employment through outside firms will be considered.
• Cannot apply to more than one job order on the same unit and shift at the facility.
• Minimal time-off requests preferred.

Unit Details:

• Uses Midas, InterCall, Curaspan, and Microsoft Office.
• Care coordination includes socially impacted discharges such as room and board/board and care placements, conservatorship, and under- or uninsured patients.
• Work includes coordination with placement agencies, Medicare/Medi-Cal guideline application for SNF and lower-level placement, and discharge planning for patients with limited resources.

Shift & Scheduling:

• Every other weekend.

Floating Requirements:

• First to float.
• Must be able to cover all adult areas of the hospital. Travelers without women's health experience will not cover women's health.
