Additional Details:
An interview may not be available prior to offer. At least one reference must be from the same unit/specialty as the job you're applying to.
Required Skills/Experience:
- Experience with discharge planning and utilization review.
Special Requests:
- Cannot have worked at any CommonSpirit, CHI, or Dignity location as a direct employee (full time, part time, or PRN) within the last 12 months; PRN employment through outside firms will be considered.
- Cannot apply to more than one job order on the same unit and shift at the facility.
- Minimal time-off requests preferred.
Unit Details:
- Uses Midas, InterCall, Curaspan, and Microsoft Office.
- Care coordination includes socially impacted discharges such as room and board/board and care placements, conservatorship, and under- or uninsured patients.
- Work includes coordination with placement agencies, Medicare/Medi-Cal guideline application for SNF and lower-level placement, and discharge planning for patients with limited resources.
Shift & Scheduling:
- Every other weekend.
Floating Requirements:
- First to float.
- Must be able to cover all adult areas of the hospital. Travelers without women's health experience will not cover women's health.
