Additional Details:

Required Skills/Experience:

• Case management/care coordination/discharge planning experience in an acute hospital setting
• Experience with facility placement to SNF, IRF, LTC, and LTAC
• Experience setting up post-acute care needs (home health, DME, IV Abx, TPN, etc.) and understanding how coverage intersects with the patient's specific payor
• Adult patients; service lines are short-stay surgical and medical

Shift & Scheduling:

• Monday-Friday
• Every other weekend required
• Remote on-call every 6th weekend, 8:00 AM-4:30 PM Saturday and Sunday
• Call Required: Yes

Special Requests:

• Clinicians who obtained their RN degree outside of the U.S. and do not hold a MO/IL/Compact license require an extended licensing and compliance timeframe
• Must be able to complete labs within 48 hours of signing
• No medical marijuana cards accepted.

Details:

• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment.
