Required Skills/Experience:

• 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.

Required Credentials:

• Dementia testing will be required prior to start.

Special Requests:

• At least one reference must be from the same unit/specialty as the job you are applying to.
• Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location before returning.
• Must disclose previous employment on your resume or elsewhere within the submission for a DNU check.

Unit Details:

• Common diagnoses/patient population include dementia, diabetes, hypertension, chronic kidney disease, falls, and residents needing ADL assistance.

Shift & Scheduling:

• Every other weekend and holiday.
• No on-call or call-back shifts.
